Our extended mission
Bridges AfterCare provides families who complete the program at San Jose Family Shelter with nine months of additional case management once they are in permanent housing.
Services provided include intensive case management and education including home visits, resource sharing and referrals. AfterCare allows families to stay connected to a supportive case manager while they navigate back into self-sufficient living. This added support model has been crucial to families now living independently.
